This page provides information for dental facilities and dentists.  At this time we do not accept credentialing applications online.  If you are interested in becoming a provider, please download the information and mail it to us.

We are searching for quality general dentists and specialists to take care of our nationwide membership base, however our emphisis is on quality of dentistry not quantiy of facilities.

Interested in replacing your fee for service patients with our plan members.	Interested in filling those hours that fee for service patients don’t want.
